Pelvic floor-related problems in women (uterine prolapse, vaginal prolapse, bladder prolapse, rectal prolapse, urinary incontinence or retention—either alone or combined—bladder dysfunction, defecation problems, or unsightly appearance of external genitalia, etc.) have presented significant challenges.

Ineffective treatment options and low success rates have troubled us gynecologists for centuries, but it is, of course, the patients who are most affected. These issues were thought to stem from muscles or prolapsed organs. Even today, hysterectomy (removal of the uterus) remains the most commonly performed procedure. Patients are still widely advised to use pessaries (rings)—a method introduced by Hippocrates—and exercises are almost always presented as the first treatment option. (This is comparable to an orthopedic surgeon advising a patient with a broken leg to perform exercises for six weeks.)

In the early 1990s, Professor Petros from Australia demonstrated that the problem lies in the ligaments and fascia, marking the beginning of a new era with his Integral Theory-based surgeries. Building on this foundation, we have further developed the Goeschen/Caliskan methods, which prioritize organ-preserving, reconstructive surgeries. These techniques completely mimic the original anatomy and physiology by repairing ligaments and fascia with synthetic prostheses, replacing outdated procedures such as:

• Highly unappealing and obliterative surgeries that close the vagina completely (colpocleisis),

• Organ and tissue-removing surgeries (vaginal hysterectomy, colporrhaphy, etc.),

• Sometimes brutal abdominal surgeries (whether laparoscopic or open) that do not allow comprehensive repair, have limited effectiveness, and are physiologically unnatural.
